Chief complaints:
A 78 year old male came with chief complaints of:
- no passage of urine from morning 
- swelling in the left hand and right leg 
HOPI:
Patient was apparently asymptomatic 1 month back when he started noticing swelling in the left ankle and gradually progressed covering the whole leg in 6 days. 
He was taken to Suryapet Hospital where they referred to Gandhi Hospital but the patient's attender did not wanted to stay there so they took him to a private hospital. 
Surgical dressing was done in view of Necrotizing fascites and patient was asked to review after 10 days. Dressing was done again and surgery was suggested. 
- From today morning the patient is complaining of no urine output and swelling in the right leg and left hand. He went to some hospital where they kept Foley's catheter but it was not properly kept . 
So patient came to our hospital with complain of Anuria since morning and swelling in the right leg ams left hand. 
PAST HISTORY
3 years back he had similar complain for which he was treated in some private hospital. 
Not k/c/o DM/HTN/Asthma/TB/CAD
PERSONAL HISTORY 
Diet : mixed
Appetite : normal 
Micturition: decreased since 2 years ( complain of burning micturition when exposed to sun) 
Bowel : constipation since 2 years
Sleep: decreased since 1 month due to pain
Habits : 
Smoking - 2 chutta/day from last 15 years 
Toddy - 1/2 litre per day from 15 years

55 YEAR OLD MALE WITH GIDDINESS AND LETHARGY

Arushi Kumari  Roll no. 151 55 year old male painter by occupation came to casualty with cheif complaints of  1.Giddiness since 2 days 2.lethargy since 1 day - Patient was apparently alright 2 days back then at 9:00 am he had sudden episode of giddiness while urinating in the bathroom where he fell down on his knees, no LOC,No involuntary movements of UL/LL. - He was taken to outside hospital found out to be having BP-250/120mm of hgand so  antihypertensives were given. - 1 day back patient was brought  here with lethargy,  decreased responsiveness for further evaluation. - On presentation to casualty , he had   BP: 180/120mm hg f/b 170/100 f/b 160/100 and 150/80 mm hg.
